CDC report links Illinois fair Salmonella outbreak to contaminated cooler ice

A CDC report on a 2024 Illinois county-fair outbreak shows how poorly maintained beverage coolers and contaminated meltwater can turn ice into a food-safety hazard.

Ice is easy to treat as a harmless part of serving drinks, but a new CDC MMWR report shows why it must be handled like food. The report, published on February 26, 2026, examined a Salmonella Agbeni outbreak associated with an Illinois county fair held from July 30 to August 4, 2024. Investigators identified seven laboratory-confirmed infections and six probable cases among people who attended the fair.

The unusual detail was that the patients did not share one meal or one food vendor. Four had not eaten at the fair at all. The common exposure was the beer tent: all 13 people who became ill had spent time in the fair’s infield and reported drinking canned beer served from the same area. No illnesses were identified among people who did not visit the beer tent.

How the cooler became a possible contamination route

The beer was chilled in a large improvised cooler made from a 10-foot length of corrugated black plastic farm drainage tile divided into four compartments. The equipment was intended to hold cans and ice, but the report says it was not food-grade equipment, was difficult to drain and had no system for monitoring its internal temperature.

Staff rinsed the cooler with a hose once at the beginning of the week. It was not cleaned with soap afterward, and standing water was later reported inside it. Melted ice was replenished each day, while different volunteers handled cans and ice with bare hands. There was no handwashing station inside the beer tent. The report also notes that the cooler was not subject to a standardized cleaning or sanitizing procedure.

The ice itself came from a local supplier using municipal water and was available to other vendors, so investigators considered contamination at the source less likely. Instead, they assessed contamination inside the cooler as the most plausible explanation. Meltwater or the outside of beverage cans could then have transferred bacteria to hands and eventually to people’s mouths.

The CDC is careful about the limits of the evidence. The cooler and remaining ice were no longer available for testing when the investigation began, so the report does not claim that contamination was definitively proven. It describes contaminated ice as a plausible source because it was the only exposure shared by every patient and because the cooler’s sanitation and drainage conditions created a credible cross-contamination pathway.

What this means for summer gatherings

The lesson applies beyond county fairs. Coolers used for drinks, picnic food or outdoor cooking can become contaminated when raw food, dirty containers, hands and ice share the same space. The CDC’s explanation of how foodborne outbreaks happen emphasizes that contamination can occur at any point between preparation and serving, including when contaminated water or ice contacts food or food-contact surfaces.

For a household barbecue or community event, the safest approach is to treat beverage service as a small food operation:

  • Use a clean, food-safe cooler that can be fully washed, rinsed and drained.
  • Keep ice used to chill sealed cans or bottles separate from ice intended to go into drinks.
  • Use a dedicated scoop or clean tongs rather than hands to handle serving ice.
  • Keep raw meat, prepared food and dirty utensils out of the beverage cooler.
  • Drain meltwater regularly and prevent cans, hands or containers from sitting in stagnant water.
  • Provide a real handwashing station with running water, soap and disposable towels for anyone serving drinks or food.

After an event, empty the cooler completely. Wash the interior with detergent and water, rinse it, then sanitize it with a product approved for food-contact surfaces according to its label and local food-safety requirements. Leave the cooler open to air-dry before storing it. A quick hose rinse is not a substitute for cleaning, particularly after a long event in hot weather.

Keep cold food cold, but do not rely on appearance

Temperature control still matters even when the immediate risk is contamination rather than bacterial growth. CDC guidance on coolers and cold food recommends keeping perishable food at 40°F or below and discarding food that has become too warm or has been held without a reliable cold source. A cooler thermometer can reveal problems that are invisible from the outside.

Do not taste food or ice to decide whether it is safe. Harmful contamination may not change smell, color or appearance. If food, ice or a food-contact surface may have been contaminated, discard the affected food and clean the equipment rather than trying to rescue it by adding fresh ice.

The Illinois outbreak also shows why event organizers should assign responsibility for cooler sanitation instead of treating it as an informal task. A written schedule should identify who cleans the equipment, how often it is checked, where clean ice is stored and how staff wash their hands. The local health department response required routine cooler sanitization and made cooler protocols part of future vendor licensing agreements.

For cooks and hosts, the takeaway is simple: ice is not automatically safe because it is frozen, and a beverage cooler is not automatically clean because it only holds cans. Clean equipment, protected ice, handwashing and proper drainage are small controls that can prevent an overlooked serving step from becoming the source of a foodborne outbreak.
