FDA Puts Root-Cause Analysis at the Center of Food Safety Prevention
A May 2026 FDA update explains how industry-led best practices and structured root-cause analysis could help prevent food-safety incidents from happening again.

The U.S. Food and Drug Administration is placing greater emphasis on prevention after food-safety incidents, rather than focusing only on removing affected products from the market. In a May 13, 2026 update, the agency’s Human Foods Program announced two new resources designed to help food businesses work with regulators, build practical safety procedures and understand why contamination events occur.
The announcement is part of the FDA’s broader 2026 food-safety priorities. The agency says its Human Foods Program is pursuing a prevention-based approach to reducing foodborne illness caused by microbiological contaminants, with closer collaboration between regulators, industry, academia, states and other stakeholders.
Best practices are meant to close the gap between rules and daily work
The FDA’s new Food Safety Best Practices for Human Foods resource explains that industry-led best practices can translate general regulatory requirements into procedures for a specific commodity or production setting. These documents may be developed by industry groups, academic experts or other organizations using experience, scientific evidence and lessons learned.
That distinction matters: the FDA states clearly that these best-practice documents are not FDA guidance. They do not replace regulations or official compliance requirements. Instead, they can provide practical detail that helps farms, manufacturers and other food producers apply food-safety controls consistently in everyday operations.
The FDA’s page lists examples covering areas such as washing fresh-cut leafy vegetables, the dry-bulb onion supply chain, controlled-environment production of leafy greens and herbs, fresh tomatoes, cantaloupes and the developing guidance work for fresh and frozen berries. The examples show how food-safety recommendations can be tailored to the risks of a particular ingredient or production method.
The agency says its subject-matter experts may contribute technical assistance, scientific information and regulatory expertise to these projects. It also encourages organizations developing best practices to contact the FDA for help with scientific and regulatory alignment.
Why root-cause analysis matters after an incident
The second resource, Strengthening Food Safety through Root Cause Analysis, focuses on what happens after a food-safety problem has been identified. A root-cause analysis is a structured investigation intended to explain not only what went wrong, but why the conditions existed that allowed the incident to occur.
The FDA describes a process that begins with assembling a qualified team. The team then defines the problem and gathers relevant evidence, which may include production records, traceability information, standard operating procedures, environmental-monitoring results, cleaning records, laboratory data and staff interviews.
- Describe the incident: establish what happened, when it happened and where it was detected.
- Map the process: review the relevant production, handling, storage and cleaning steps.
- Analyze possible causes: use hazard analysis, process maps, flow charts or cause-and-effect diagrams to identify contributing conditions.
- Identify the underlying cause: document why each possible cause was accepted or rejected.
- Correct and verify: apply immediate fixes, introduce longer-term preventive measures and check that those measures work.
This approach is different from simply identifying a contaminated batch. A recall can remove a known product from sale, but an RCA is intended to reduce the chance that the same failure will happen again. The FDA says the findings can also contribute to industry-wide best practices and inform future regulatory guidance or oversight.
What the FDA is doing internally
The agency says it has reviewed root-cause-analysis practices used by food businesses and regulators around the world. It also conducted retrospective pilot analyses of past incidents using a new standard operating procedure and electronic tools during 2024 and 2025.
In addition, the FDA has participated in industry discussions and updated its Investigations Operations Manual with sections covering root-cause investigations. The agency’s description emphasizes comprehensive data collection, facility evaluations and detailed documentation when investigating foodborne illness outbreaks.
The work supports the FDA’s wider Human Foods Program priorities for 2026, which identify microbiological food safety as one of three central areas alongside food chemical safety and nutrition.
Why this matters to people who cook
For home cooks, the announcement does not introduce a new kitchen rule or change how everyday food should be prepared. Its importance is upstream: stronger procedures for suppliers and food producers can improve the way ingredients are grown, processed, transported and monitored before they reach a kitchen.
It also reinforces a useful principle when following a recall or safety alert: the immediate consumer instruction remains the priority, but the longer-term goal is prevention. Businesses should not stop at identifying the affected product. They should also determine which process, supplier, sanitation step, training gap or verification failure allowed the problem to develop.
The FDA describes food safety as a shared responsibility and says the new resources will continue to evolve with scientific knowledge and lessons learned. For the food industry, that means more emphasis on documented evidence, repeatable procedures and verification. For consumers, it offers a clearer explanation of how regulators and food businesses can turn individual incidents into preventive improvements across the supply chain.
